How to register

St. David’s Surgery is a long established, small and friendly practice serving St. David’s and the surrounding area. The Practice boundary covers roughly a 10-mile radius from St. David’s and extends in an arc from Mathry to Newgale. This area includes most of that covered by the postcode SA62 6.  If you are in any doubt about whether your address is covered by us, please ask and we can advise.

Please also complete this brief health questionnaire along with the Registration Form and return both to us.

Your medical records should be forwarded to us from your previous GP Practice within a few weeks.

We recommend that all new patients have a health check with a Practice Nurse as this enables us get to know you and your medical history and any medicines you may be taking.

Do I need to provide ID in order to register with a GP?

When registering with a GP, it is helpful if you can provide photo ID (passport or driving licence) AND at least one of the documents below:

  • A recent utility bill or proof of utility account, to show proof of address
  • NHS medical card
  • Birth certificate
  • For patients from overseas – a biometric residence permit/ a Home Office ‘travel document’ / or an ARC asylum seeker’s card
