Community Nurses (known as District Nurses)
The Community Nursing team provides professional nursing care to patients who are housebound. Following an assessment, they will prescribe appropriate nursing care and treatments, obtain nursing equipment, liaise with other health care professionals (principally the patient's GP) and give support and advice to families and carers. Their role includes the care of patients following early discharge from hospital, as well as palliative care and symptom management.
