Health Visitor

Our Health Visitor team is based in Fishguard, but attends the Practice weekly to provide families with advice and information on all aspects of health care for the assessment and development of children under five; they have access to a wide range of information and services which you may find useful. Community Nurses (known as District Nurses)

The Community Nursing team provides professional nursing care to patients who are housebound. Following an assessment, they will prescribe appropriate nursing care and treatments, obtain nursing equipment, liaise with other health care professionals (principally the patient's GP) and give support and advice to families and carers. Their role includes the care of patients following early discharge from hospital, as well as palliative care and symptom management. The Community Nursing team for St. David's is based in Fishguard, and can be contacted directly during office hours on 01348 430930
