How to register

St. David’s Surgery is a long established, small and friendly practice serving St. David’s and the surrounding area. We have an open list meaning we welcome registrations from patients already living in or moving to the practice area. The practice covers an 10 mile radius from St. David’s and extends in an arc from Mathry to Newgale. This area includes most of that covered by the postcode SA62 6. If you are in any doubt about whether your address is covered by us, please ask and we can advise.

You can register by completing a simple NHS Wales form which is available from Reception (we can post this to you on request).  Alternatively, you can print the registration form here and return it to us >> Download the Patient Registration Form .

Please also complete this brief health questionnaire along with the NHS Wales registration form and return both to us.

Your records should be forwarded from your previous practice within a few weeks.

We recommend that all new patients have a health check with a practice nurse as this enables us get to know you and your medical history and any medicines you may be taking.
